This program provides individualized education and counseling for patients with Type I and Type II Diabetes. Nurse educators work with you to cover diabetic self-care instructions for diet, medication management, and instruction on home glucose testing. The Nurse Educator also assesses the need for referrals to Podiatrists, Ophthalmologists, and Nutritionists to provide early detection and care for complications such as eye problems or foot wounds.

Sweet Success Program
The Sweet Success Program cares for pregnant women with gestational diabetes, to prevent possible complications such as large babies, higher C-Section rates, and higher recurrence of diabetes later in life. The program is currently available at Eastmont Wellness Center. Please ask your health care provider or clinic nurse to make a referral for you.
