- Applying for this job authorizes Alameda County Medical Center to contact any of your schools, your current* and former employers, or other references for the purpose of collecting information and/or obtaining an account of your work experience and skills. Please note that, if an offer of employment is made, a more extensive background check is part of the employment decision-making process and you will need to sign a "Disclosure and Authorization to Obtain Consumer Report for Employment Purposes" form as part of this pre-employment process.

- 6. Employment Eligibility Verification
- All offers of employment by Alameda County Medical Center are conditioned on the provision of satisfactory proof of your identity and legal authority to work in the United States. Prior to your first day of employment, you must comply with the requirements of the Immigration and Naturalization Service's Employment Eligibility Verification (Form I-9).
- 7. Post Offer, Pre-Employment Medical Examination
- Alameda County Medical Center requires a limited medical and screening examination and clearance from the provider designated by your potential prospective employer. This initial exam will be provided to you at no charge. Your exam will include an assessment according to the standard procedures of Alameda County Medical Center, as needed, based on pre-employment requirements, the physical requirements and essential functions of the position you are being offered, and the degree of patient contact. An exam would typically include (but may not be limited to): TB skin test(s) with a follow-up exam in 48-72 hours; a limited mobility test and eye exam; and a blood draw (unless you have acceptable, recent verification of current vaccines for Measles/Mumps and Rubella).
- In order to ensure a successful exam, please bring your current vaccination records, prescription drug information, corrective eyewear/prescription, and your position description (which you should obtain from your prospective supervisor). If you have any disabilities, you should voluntarily make this information known to the exam provider. This information, as well as other aspects of your exam, are kept confidential.
- If you do not receive medical clearance, you will be advised of that fact by the exam provider, or the employment/human resources representative. In such a situation, you will not be eligible for employment in the specific position for which you have applied. If applicable, you should contact the Employment Office/Human Resources Department to discuss whether a reasonable accommodation can be made.
- By submitting an application for consideration, you acknowledge your understanding and agreement that failure to successfully complete the required post-offer, pre-employment medical/screening exam will result in taking back (rescinding) the employment offer or terminating your employment.
